1. Sustainability program review (Must check box)
We have reviewed the GSA sustainability principles, implementation criteria and work streams.

We are starting a new project that addresses one or more of the work streams.
We have an existing project that addresses one or more of the work streams.
We have other projects that substantially address the guidelines.

	We would like to submit a project plan and annual report to the GSA regarding the activities and results for at least one project that addresses the work streams.
	We would like to submit a project plan and annual report to the GSA on all of our policy, practice and activities that substantially address the work streams and guidelines.

Annual reporting will be in a simple format based on the guidelines. Reporting helps GSA to demonstrate industry progress!

4. Collaboration (Optional activities)

	We are interested in presenting the activities and results of our project(s) at a GSA event.
	We are interested in sharing success stories in the GSA newsletter or other publications.
	We are interested in volunteering on the GSA sustainability working group to continue the development of the guidelines and work streams.
	We are interested in participating in grant applications that may benefit our project.
